It's easy to request your waiver on-line, by clicking on Waiver Request.

Waiver Request

To access our Waivers web application, the "Log In / Registration" web page will require a valid user id and password.
Login
Enter your UserID and password, and then click the "Login" button.
Did you forget your User ID/password?
If you cannot remember your UserID and/or password OR if you are converting your old e-mail-based login to the new UserID-based login then use the 'reset account' feature. Enter your E-mail address and click the 'Reset Account' button - we will send you a new UserID and password.
First time User?
If you do not already have a UNICOR online account, please create one by typing your e-mail address and clicking the 'Register Now' button.

Note: If your waiver request includes more than 10 items, please fax it to the
UNICOR Customer Service Center at 859-254-9390 or 859-255-3703.

To enter your waiver request on-line, please provide the following information:

Customer Reference Number
*Justification
The following line item information is needed:

*Description

*Quantity

*Unit Price

The on-line waiver request process includes 2 steps.
1.
Data Entry.
Fill-in the blank waiver request form. Then click the "Submit Waiver Request" button. If your waiver request has been completed successfully, a Review Waiver Request web page will appear.
2.
Review.
Review your completed request for accuracy. Then click the "I Agree" button. Your waiver request will be entered for processing, and the UNICOR Waiver Request Number assigned will be displayed.
